If there is a discrepancy in the tattoo name on the consent form and when it is not completed (such as if the tattoo had already changed), or if the tattoo will need to be re-performed after one or more layers depending on the tattoo base chosen, we will then call a body to confirm there is an issue and have our staff take a look at it and take the appropriate follow-up actions for any discrepancies (such as contacting the customer for clarification if needed) in regard to the consent form. If at that point we still need to contact the body to have the consent form processed with further follow-up actions, then that is okay, as we still need to have the consent form processed. It will be a better tattoo experience and our customers will feel more comfortable about their tattoo choices and their bodies. It will also make the process of moving through layers and the tattoo base more efficient if we are in a better position to help them out.

You are advised to contact all clients you have tattooed at each of our locations and inform them of the tattoo counseling required before they attend any In Person (IN) tattoo counseling with you at one of our locations. This step is required by law. Can I bring my own tattoo items with me? You are advised to bring all applicable tattoo items that you would like your tattoo done.
